My husband and I developed 'Family Health Notes' after years of helping our parents through difficult times. For our own benefit and more importantly, the benefit of our parents, we realized the critical importance of keeping medical information in a convenient booklet. Our conclusion was formed after hundreds of hours in medical facilities, answering or worse, guessing responses for many of the questions. How long ago were the lab tests? When was surgery performed? What prescriptions are you taking? A booklet that provides a place to store notes containing physician names, prescription data, appointment notes, surgery notes, lab tests, and on and on became key to our survival!

In emergencies or for regular visits to the doctor's office we grab our 'Family Health Notes' booklet so when we arrive at the medical facility, filling out forms and answering questions are effortless and a time saver in critical situations. We also learned how vital accurate medical information is to the physician.

The care of our family members is a high priority and at times complex. Anything to organize and save time is precious and has value that often can not be measured in just hours and minutes. Family Health Notes has provided us with an additional level of assistance to our parents and has provided our parents with an organized resource of their medical records. We have also seen the comfort level our parents have experienced by having ‘Family Health Notes’.

Our decision was and is to give the best possible assistance to our parents just as they provided us throughout our life ... ‘Family Health Notes’ helps us to provide that assistance.

We hope your visits to the doctors and hospitals are few and far between but we recommend using a ‘Family Health Notes’ booklet for each member of your family.
